Why metformin became a longevity candidate

The interest started with epidemiology — observations that people with diabetes taking metformin sometimes did better on certain outcomes than expected. A randomised study in older adults found that metformin reduced some of the mitochondrial adaptations to aerobic exercise training — in other words, it may partly interfere with the benefits of the single most evidence-backed longevity behaviour there is.[2] If structured training is central to how you invest in your health, this is a real tension, not a footnote.

The evidence gap for healthy people

Most of the useful metformin data comes from people who have diabetes or are at high metabolic risk. Extending those findings to metabolically healthy adults is an assumption, not a demonstration.
